Office Policies

The following policies help us maintain a high standard of care for all patients. Please review them prior to your visit.

Forms & Required Submissions

To provide the highest quality care, patients are required to complete requested new patient intake forms, follow-up forms, and other required submissions prior to appointments, when applicable. Failure to complete required forms before your appointment may result in delays in care or the need to reschedule.

Medication Refill Policy

We encourage patients to discuss medication refills during scheduled appointments whenever possible so we can provide safe, thoughtful, and uninterrupted care. When clinically appropriate, refill requests may be accommodated between appointments, including in certain urgent situations. However, controlled substance refills require an appointment and cannot be provided outside of a scheduled visit.

Communication

For non-urgent matters, patients may contact the office through the patient portal or other designated communication channels. Current patients may also communicate with our office by text for routine matters, and many find this method to be convenient and helpful. Please allow appropriate time for responses, as messages are reviewed during regular business hours.

Rescheduling

Requests to reschedule should be made at least 24 hours in advance. Repeated late cancellations, no-shows, or last-minute rescheduling may affect future scheduling availability.

Late Arrivals

If you arrive late, your appointment will still end at its originally scheduled time so that other patients may remain on schedule. Patients arriving more than 15 minutes late may need to reschedule and may be considered a missed appointment.

Referrals

Some insurance plans require a referral from your primary care provider before your appointment. Patients are responsible for confirming whether a referral is required and ensuring that it is obtained prior to the visit. Failure to obtain a required referral may result in claim denial and patient financial responsibility.

Forms, Letters, and Administrative Requests

Requests for forms, letters, record review, or other administrative services completed outside of scheduled appointments may be subject to an additional fee, depending on the time and complexity involved. Whenever possible, please discuss these needs during your appointment.

Medical Records

A signed authorization is required for the release of medical records to another provider or third party. Patients requesting completion of medical paperwork or special record preparation may be subject to an administrative fee.

Urgent Concerns

We make every effort to address urgent clinical concerns as promptly as possible. However, same-day or immediate appointments cannot be guaranteed. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency room.
