Request an Appointment
Begin by requesting an appointment through our website or contacting our office.
Once scheduled, you will receive access to your patient portal along with any required intake forms and questionnaires.
Complete Intake Forms
To help us prepare for your visit, please complete all required forms prior to your appointment.
- Medical and psychiatric history
- Current symptoms
- Rating scales (when applicable)
- Consent forms
Completing forms in advance allows us to make your visit more focused and efficient.
Attend Your Evaluation
Your first visit is a comprehensive psychiatric evaluation, typically up to 60 minutes.
- Review your symptoms and concerns
- Discuss your history
- Answer your questions
- Develop an initial treatment plan
Our goal is to ensure you feel heard, understood, and supported.
Ongoing Care & Follow-Up
If ongoing care is recommended, follow-up visits will focus on:
- Monitoring progress
- Adjusting medications if needed
- Reviewing symptoms and treatment response
- Providing continued support
Care is individualized, and treatment plans are adjusted over time based on your needs.
Before Your Appointment
- Complete all required forms in advance
- Ensure you are in a private, quiet location for telehealth visits
- Have a list of current medications available
- Be prepared to discuss your symptoms and concerns
Telehealth
Most visits are conducted via secure telehealth.
In-Person
In-person appointments may be available upon request.
Communication
For non-urgent matters, patients may contact the office through the patient portal or designated communication channels. Many of our patients also find text communication to be a convenient and helpful way to stay connected with our office for routine matters.
